I see duplicate tickets are showing when I create a change request.
Navigation as follows.
1) I created a ticket by selecting the type as request -->Ticket
2) Then I decided that it should be a change request so I have a created a change to the existing ticket.-->Change
3) assigned to the technician and created couple of tasks.-->Tasks
4) When I login with the technician I see two Changes with same ID.
Note: When I see from administrator login i see only one change.(I tried modifying the profiles also but no luck)
How to avoid duplicates under change creation of tickets
5) I am able to modify the change even after status is in closed state.
Restrict the modification to the closed change

Case 1:
Today I observed that I see 3 duplicate changes from one technician. This technician is the requester for that change and the change has been assigned to two other technicians including the requester.
For other technicians it is visible only as one change with ID 41
Where as requester/technician is able to see three changes from his login 3 duplicate ID
Is it a Bug in version 9.1.2

I can't reproduce your issue event doing all your action.
I only have one change created
For closed change, you must lock it in the cycle life of the changes in your profile

Hi Yellen,
Further to your query Duplicate visibility at change level.
1) Technician is the one who created the change(so he will be as requester) and the same technician is also appeared at assigned to.Then same change is assigned to another technician for development and once again assigned to Database team,
Now the change visibility at requester level appears as 3( even though in database it appears as 1).
I have added an additional filed at Change level using plugin . That is for estimated hours (duration).
My profile doesn't have the option to reopen the change at life cycles.I restricted that.
But I have enabled the update rights at change level. I have enabled update level because I can't see the ADD task tab if I remove the update rights at profile level.
