New fields in helpdesk.html

Is possible to include new fields, like name, phone, address in the helpdesk form?

I want to include GLPI as a service to put new tickets to people who register in my web (joomla based).
The helpdesk.html from doesn't include information from the user.

This is I want to do.

user is registered to my joomla page ---> redirect to support page---> in this page the user could be capable to create new ticket, check ticket status.

Is there any way to do this?
