Is it possible to add a header to the pdf when you export/print?

I go to Inventory --> Computers
There is a long bar that states "Current Page in PDF"

I use this. This saves as a PDF, but there is nothing on the PDF to tell me where it came from.
The above should have a header or footer that states "Computer Inventory" or something.

When I try to use the PDF plugin, the new window opens but there is never any data; it's always blank.

** EDIT **

My issue with the PDF plugin is having the problems box checked. Having that checked, will not return a pdf.

Hy,
How can I add a text to the header of the print tp pdf plugin.
Ex. Now it looks like that : TypeName- Name, Entity
I want to be like : Name of the report, TypeName-Name, Entity
