heroes~~ help for error-400&500

here s error.log , please help me to analysis the reason why always can not run to the end  thanks everyone ~~

when I check the apache log folder find the following error in error.log :

Issuing rollback() due to DESTROY without explicit disconnect() of DBD::mysql::db handle database=ocsweb;host=localhost;port=3306 at C:/xampp/perl/site/lib/Apache/Ocsinventory.pm line 61

why ???
