GLPI FormCreaor Error

I need to create the following table inside a form...

r"https://ibb.co/2F1yvTD"

Where the fields "Accounting Classification" and "Measure" are dropdowns. however the result is as follows...

r"https://ibb.co/QJ2CssN"

It returns all the options inside "Accounting Classification" and "Measure" instead of returning the selected option. I would like to Know if there is a way to fix this.
