SMTP for entities

Please explain to me what was the concept of entering an email address for notifications in entities if you cannot set up an smtp server and password... If I understand correctly in most cases (if the email address is different as the main email for notifications) it ends up not sending notifications - the mail server rejects the email. How should this be configured?
