FORMCREATOR - GLPI 10.0.9 - Form does not appear in the assistance tab

"I created a form in 4 different GLPI installations, but in only 1 of them, the form appears in the assistance tab (it appears as created but the form does not appear in the assistance tab to create the form).

Can someone help me?"
