Documents are attached as separate comments

Hello. Recently I discovered such a problem: when attaching documents to a comment of an application, 2 comments are created: 1 with text and with an attachment. How can this be fixed?

Guys, hasn't anyone encountered this problem? In earlier versions, this was definitely not the case. Tell me where to dig.

What version are you using?
I don't think this is a bug, other than the document been missplaced to the left

Previously, everything was fixed in 1 post, now there is a spread across different messages.
