New ticket form

I need to create a new form for my telephone attendants containing only requisitor, assigned and a description of the call.
How do I edit the form template to only these three fields appear for my attendant?

go to ticket > manage templates > default >

you can custom your ticket form by modify the hidden fields and see them from tab next to hidden fields
