Add Templates

I would like to know if with the pluggin form creator I could design a template for my tickets, in fact I already do it, and I need to know how I can add the template to can be visible in ticket templates or how I can use it??.

GLPI version GLPI 0.83.1

I already read the topic, but I dont know how to do for the form I design with form creator can be visible in ticket templates.

I configure the form with a category, but this not make it appear in ticket templates.

Can somebody tell me after I made a form with form creator, how to use it???

I add a picture to be more clear:
1. I already made the form as follow:
[img]C:\Documents and Settings\cgiraldo\Mis documentos\Mis imágenes\Imagen\glpi1.png[/img]
2. The form is active
3. I configure the form with a specific category

Somebody can tell me how can I use it?? In templates dont appear, and I dont have idea how can I use it

I already find how to use it!..

The form created with the plugin form creator, don´t appear in the templates, but after its configuration the user can fill the form that appear in the same place of plugins, but by automatic is created a ticket with it.

Now I have a nex question.   The ticket was created but i dont know where I can see the fields that have been personalized in the form, I see in the pluggin and there is only the form, it doesnt have the information added, like by example other plugin like objet management.  I see in the ticket created, but here dont see the fields personalized either.

Some body can help me ????
