Helpdesk for users Post-Only

Friends, 
 
It would like a help to modify the helpdesk page for users with profile Post-Only 
 
I have friends that know PHP.

I need: 

1 - To remove the priority level from page for users Post-Only
2 - To remove the word hardware type of the table and to move the word My Devices and your respective dropdown for your place 
3 - Organize the icons of the header and to place a big title. example.: <h2 align='center'>Form for opening of technical calls </h2>
4 - Increase the size of the field e-mail for at least 30 characters
 
I need to know how that page is generated PHP to modify the attributes . I believe that when understanding as the page it is created, everything is easier
 
Does anybody can help me?
