Depreciation in 0.78

I'm currently working with Depreciation in 0.78.  I'm using this as a reference for the different items I'll need:

Could someone point out what actual values are needed for Decreasing depreciation to work?

Here's my assumptions:

Values needed for depreciation
Cost, purchase date, first period date, salvage value, useful life, rate, basis

MAP:
Cost of asset = Value field
Purchase Date = Date of Purchase
First period date = ? (Startup Date?)
Salvage value = ? (maybe Warranty ends value?)
Useful life of asset = Amortization Duration
Rate = ?  (Some default value, I'm guessing?)
According to the article, for 5 or six year depreciation cycles, use coefficient of 2 = Amortization Coefficient
Basis = ? (Default value?)


So, my questions are the fields above where the question marks are. Where should I put first period date and salvage value? What are the default rate and Basis values?  I did look through the wiki and the forums and didn't see many topics that have to do with depreciation.


Now using 0.78.1 on CentOS.
