issuing item form

Is there any plugin for issuing computers, mobile phones ect.. to people in the company.
Simple form that connects items in inventory with contract, but with ability to print form out.

Thank you for your answer ,but maybe you did not understand me well. However, i need form where will be item from GLPI (computer, phone monitor ..ect.) user name, date and signature from user that he took this equipment.
