Business Rules not fired

We are facing problems with some rules not being aplied here.
When the ticket is created, we have business rules: to assign the ticket to group, to add requester groups, and to send an aproval request. But sometimes, even the ticket being created,  this rules has not been  applied and we have to apply the rules manually,
But when we use the “test rules engine” option, the rules are fired and we can observe that this problem is sporadic.
Any ideas?

Did you check ticket journal ?
Is you business rule on create action or update ?

If the user create ticket and fill on option after the creation this could be this situation.

For business rule, all rules are applied.
Have you other rules after your rule described in your post?

I found out a solution here.
I have created a stored procedure which perform all operation we need and then an event to call this stored procedure.
I know that isn't the best solution but its enough for now.
