GLPI solution template

Hi Guys. I'm new to the forum and I'm sorry for my English!
I need to do a check list as a solution template in GLPI.
I did it by placing the following code in the source code section:

<p> <strong> Review of PC delivery </ strong> </ p>
<form action = ""> <input name = "gender" type = "radio" value = "male" /> Office Tools <br /> <input name = "gender" type = "radio" value = "female" / > Basic Applications <br /> <input name = "gender" type = "radio" value = "other"; /> Other </ form>

It shows me correctly check and I can check but the problem is that when I save the solution, the checks are not saved.
